Our Story

We're really healthcare nerds at heart. Actually, we're all the kinds of nerds. Our small team of three met in a little fantasy village called Cherrytown playing Dungeons & Dragons with our local group Circle of Barhaven. After taking on epic quests and challenges in our fantasy world, we realized if we could make such an impact together in a game, why not in the real world? 

Our journey from rolling dice to rolling out our concierge clinic was driven by one shared belief: everyone deserves excellent, accessible, and affordable healthcare!

Hi! I’m Dr. Mia Guzman.

I am a primary care doctor, board certified in Family Medicine and licensed to practice in Texas. I established c.a.l.m. Healthcare to help patients get the care they need fast and from the comfort of their home. 

 

With over a decade of experience in hospital and clinic settings, I found many health concerns can be effectively addressed via messaging or video. Weight management and primary mental health care are a priority for me, so I am making them more accessible to everyone.

 

I’m a patient, too. I understand getting care can be a hassle. I avoid going into the office or hospital. I dread the ordeal of making an appointment, taking time from work or my family, waiting in the lobby, waiting in the exam room, and answering the same questions multiple times. I briefly see the doctor or provider, then finally get what I came for.

At c.a.l.m. we skip all that. You subscribe to message me directly. We provide care and continue to respond to your needs for a low monthly fee. Healthcare can be complicated. We strive to keep things simple. It’s real healthcare, from me, an experienced board certified physician. The same care you would get at my primary care office.

Our current service focuses on medically supervised weight management and primary mental health care topics: major depressive disorder and generalized anxiety disorder. We can help with any medical issue that can be addressed online.

  • Can I cancel my membership?
    Your subscription can be canceled any time through your member portal on this website (click on your name on the top of the website after you login).   • What does your service cost and what is included?
    We charge a subscription fee of $50 per month. That is the only bill you will ever get from us. This includes diagnosis, treatment plans, and the ability to send messages and receive answers from your doctor. We do not charge your insurance and we do not charge copays or visit fees of any kind. We provide medically supervised weight management with prescriptions, advice, and monitoring. We also diagnose and treat major depressive disorder and generalized anxiety disorder. Although these are our main focuses, we can help with any other medical concern that does not require an in person visit. Feel free to ask via messenger after signing up! We order blood work and continue to monitor and adjust medications. We regularly review our subscription prices to be as low as we can make it. Our competitors typically charge $70-100 per month for their subscription fees, then charge you in advance for medication. Their fees typically price medication into their plans such as $280 per month all inclusive ($80 for subscription, $200 for prescription), but may not be transparent about goal dose cost ($400/month for semaglutide for eventual total of $480/month). Our competitors upcharge you on the medication as well, but we have you purchase directly from the pharmacy to save (A LOT) of money. For example, one of our competitors charges about $200 for a month of initial dose semaglutide. Our pharmacy has the same available for $88. Their max dose charge is about $400, while a month of our max dose is around $190. Healthcare should be affordable, so we keep our subscription fee competitively low and can shape an effective care plan around most any budget. General health advice in our information section is always free and is updated regularly!
  • What is the cost of semaglutide injections?
    The semaglutide we prescribe is sold directly by the pharmacy so we don't add on extra fees at all. The chart below gives the approximate pharmacy costs for semaglutide injections: *Our preferred pharmacy ships each order with all supplies and instructions included in the price. Shipping costs are not included in the price. Shipping cost up to $30. Once on maintenance dose can ship multiple months at once. Pickup in Northwest Houston is free.
  • What is the cost of Tirzepatide injections?
    All medications we prescribe are sold directly by the pharmacy and we don't add on extra fees at all.   • What is your privacy policy, HIPAA compliance information, and user agreement?

  • Do I have to be on medications for life?
    Not always. Sometimes when we reach our goal weight or have stabilized our depression treatment, we can trial off of medications for a month or two under supervision. If you do well, we recommend regular check-ins to see if it would be beneficial to resume medication. We only prescribe medications that have been found to be safe and effective long term. This means if we do need to stay on medications to maintain your goal level of wellness, we can do so without issue.
  • What does c.a.l.m. stand for?
    C.a.l.m. stands for concierge asynchronous licensed medicine. Concierge medicine is subscription based medicine that allows a physician to have a lower patient panel to better focus on and cater to each patient. Asynchronous medicine is a newer concept in healthcare that is care occurring at a different pace, where you send a message, then later receive a reply, rather than a conversation in real time. We are a hybrid model that starts off asynchronous, but we have the capability to reach out for a video or live chat if needed. Licensed medicine indicates that Dr. Guzman and any covering physicians or clinicians for our service are licensed to practice medicine in Texas and board certified by their respective professional societies. These two items ensure that you are receiving quality care for real physicians who are held accountable for the proven efficacy of their medical advice and treatment plans.
  • Why is your cost lower than similar services?
    We are an independent clinic and we keep overhead low. There are no share holders to pay like a publicly traded company. We don't have non-clinical business owners like a spa or boutique. We are a physician led medical clinic designed to help you, not extract profit. Your competitively priced subscription fee gets you direct access to me and my team, not salaries for businessmen and investors.
  • Do you require insurance?
    We do not require insurance. We don't even bill your insurance if you have it! Your membership fee covers direct access to Dr. Guzman including medical advice and prescribing of medications when indicated. Beyond your membership fee, we will not charge anything else. Insurance may be helpful to cover any blood work, medications, or specialty referrals. The medications we choose and the pharmacies we recommend tend to be the lowest cost available for both insured and uninsured clinic members.
  • Are my answers private?
    All information you share in the app environment is confidential and subject to the Health Insurance Portability and Accountability Act (HIPAA) which ensures the highest standard of patient privacy and securing of your protected health information. By becoming one of our patients, we are establishing a therapeutic relationship which is subject to doctor-patient confidentiality. Your replies will never be used for any purpose other than to help you meet your healthcare goals.
  • How does the free trial work?
    When you sign up for the first time, you get a 2 week free trial to check out our services. Medical treatment is an ongoing process to be successful, so we're happy to get you started and welcome you to stay for a paid subscription if you like the way we operate. At the end of your 2 week free trial, your subscription will begin charging to your card on file. Your subscription can be canceled at any time through your member page on this website (click on your name on the top of the website after you login). Canceling is easy and does not require our approval or a phone call.
  • What is the cost of oral semaglutide?
    We prescribe oral semaglutide directly from the pharmacy and do not add on any fees. The pharmacy charges $250 for a 30mL vial. A dose is 0.5mL under the tongue daily for 2 weeks and then 1mL daily max dose. Oral semaglutide is not as fast acting as semaglutide injections.
  • What does medication and blood work cost?
    Not included in our $50 fee is coverage for blood work costs or medication costs. These may be covered by your insurance and you can find out what your out of pocket costs will be through your pharmacy or Quest labs if blood work is ordered. Most standard medications and blood work are covered by insurance plans under the diagnoses we use. Cost will vary based on your coverage. We will always work with you to find a treatment plan that fits your budget. Examples of out of pocket medication prices: Compounded semaglutide, currently the most effective weight loss medication, typically costs $90 per month out of pocket up to $200 per month out of pocket when you get it from our partner pharmacy. Our competitors charge around double that price, sometimes more. Brand name Ozempic or Wegovy can sometimes be covered by insurance, but this is rare. Their out of pocket costs are generally over $1000/month, so we do not typically prescribe them unless coverage is assured. Alternate appetite suppressants topiramate and bupropion (generic Wellbutrin) are affordable at cash prices of $11/month to $20/month through HEB, Walmart, or Walgreens. Depression medications cash prices range from $5/month for generics to $50 or $100/month for newer antidepressants. Insurance will sometimes cover this cost depending on your plan. Blood work prices out of pocket can be found on https://www.questhealth.com/shop Insurance coverage varies by plan, but is typically similar to costs you would incur in any doctor's office. Talk to your insurance provider to determine eligibility for coverage.
  • What is semaglutide and how does it work?
    Semaglutide is known as a GLP-1, glucagon-like peptide 1, receptor agonist. It acts like a chemical that is normally released from the gut in response to eating. It promotes insulin release in the body which reduces blood sugar levels in diabetics. It also delays gastric emptying which causes people to feel full after eating less and for a longer period of time. This promotes weight loss seen with GLP-1 receptor agonists like semaglutide.
