Your Intake Form

After subscribing to a plan, click the button below to fill out an intake form. You will be directed to our HIPAA secure host to fill out basic information and have the option to fill out any of our surveys.

Message the doctor

Follow the link below to sign up for HIPAA secure messaging through our platform, Klara. Once you sign up and link it to your mobile device, messaging can be accessed here or by text messaging 832-415-1334 at any time. Replies from the doctor and prescriptions require an active paid membership. This is your direct line to the doctor. Messages are answered during business hours Monday - Friday.
